Lumbar and thoratcic mri result could someone please help?

here are my results hypertintensity ont1andt2 involving the sacral segment mostly reflective of several focal hemangiomas l3-l4disc bulg and facet arothropathy broad base disc protrusion at l4-l5 central and bilateral neuroforaminal stenosis broad base disc protrusion at l5-s1 central c stenosis and narrowing on right neuroforamina thoracic degenerative changes on vertebral body endplates on lower and mid thoracic spine with modic changes narrowing and modic changes at c5-c6 and t7-8-9-10 levels in throacic spine do i need surgery i also have foot drop on left side as well have a brace for that can anyone help me to understand this alot of pain on med and have done pt no help
